Hotel Request Form

Welcome to the Hotel Information Request form:

This form is intended for SA club officers to use, to request Hotel information.

Please fill out this form to request Hotel information. SA requires that its clubs use the approved travel agency for SA. This form is only a request for an ESTIMATE, not the actual booking.

Please Note:

  • The entire booking process generally takes about 3 weeks.
  • Pricing and availability change constantly. The earlier this form is submitted, the better off your club is.
  • SA has MANY constraints on the ability to book accommodations. We do the best we can under these constraints.
  • SA is not able to just book the same travel arrangements at the same prices that are offered through online services.
  • SA has to book via our approved travel agency, and options may vary from what you have found online.
  • SA will attempt as best as we can to match your desired itinerary, but some changes may be unavoidable.
  • Please use expedia or hotels.com to research your hotel options
  • Once booked, hotels are NOT REFUNDABLE
  • It is your responsibility to research the hotel location you are requesting, and the quality of the neighborhood
  • The person checking-in must be 21+
  • We cannot book AirBnB's

 

Timeline:

  1. Request your ESTIMATE using this form.
  2. Once you have your estimate, submit a request funds through SAFE
  3. Once the request for funds has been approved, inform SA that you are ready to book.
  4. If the original travel options are still available, at the approved amount you requested, the request will be confirmed with the travel agency.
  5. You will receive confirmations when everything is booked.

*All time frames stated are estimates. No turnaround time is guaranteed.

What do you need to know before you continue?

  1. Contact info for yourself
  2. Hotel location
  3. Arrival and return dates and times
  4. Number of people traveling
  5. Reason for hotel
  6. The legal name of each passenger (as it appears on their ID)
  7. The birthday of each person staying at the hotel
  8. The sex of each person staying at the hotel

Please submit this form and an estimated cost will be provided to the club so a request can be submitted in SAFE. You need to request the funds and have an approved PO for your reservation before the hotel reservation will be made by SA.

Hotel Info:


Please share your research on hotels with us. We need you to provide us with at least one screen shot of a hotel that would be most ideal for your situation. You can include up to 4 hotels incase your first option is not available.
